Bell’s palsy is the most common disease of facial nerve and defined as an idiopathic unilateral facial nerve paralysis, usually self-limiting. However, several conditions like as brain tumor, Lyme disease and lymphoblastic leukemia also cause facial nerve paralysis. A few reports present that other disease mimicking Bell’s palsy. We report a patient who initially diagnosed Bell’s palsy, but later diagnosed acute facial palsy due to non-Hodgkin’s lymphoma originating from breast.

Objective: Adenosine deaminase (ADA) is the catalysing enzyme for the deamination of adnosine to inosine. CSF ADA is thought to be released by T lymphocytes during cell mediated immune response such as tuberculous meningitis. We studied CSF ADA level in various meningitis patients.
Methods
We assayed the activity of ADA in the CSF of 162 patients with various types of meningitis from May 2008 through May 2012 at Daegu Catholic University Hospital. All patients were received CSF study involving ADA level and brain imaging. The patients were separated into tuberculous meningitis, non-tuberculous meningitis and aseptic meningitis groups.
Result
Numbers of tuberculous meningitis, non-tuberculous meningitis and aseptic meningitis patients were 17, 28 and 117. Mean values of ADA level in tuberculous meningitis, non-tuberculous meningitis and aseptic meningitis were 12.26±4.41 IU/L, 9.43±16.9 IU/L, and 3.26±2.91 IU/L. The CSF ADA was very high in 2 patients of severe non-tuberculous meningitis (61 and 77 IU/L). The cut-off values of CSF ADA level for diagnosing tuberculous meningitis was 10 IU/L. The mean ADA value in CSF was higher in tuberculous meningitis than in other meningitis. The sensitivity of the test for diagnosing tuberculous meningitis was 0.82 and specificity, 0.97. Increased CSF protein in tuberculous meningitis (175.17±61.9 mg/dL) showed in relative linearity with ADA activity in CSF (p<0.05).
Conclusions
We thought that CSF ADA level may help in early diagnosis of tuberculous meningitis. However, sometimes higher ADA level can observed in severe bacterial meningitis.

Histiocytic necrotizing lymphadenitis is a benign disease of the lymph node and spontaneously recovered within 4 to 6 months. Involvement of central nervous system rarely occurs as a form of aseptic meningitis. A 20 year-old woman presented with fever and headache. CSF studies revealed pleocytosis and elevated protein levels. Her symptoms were improved without specific management, but after several days generalized edema, multiple arthralgia and painful lymph node swelling with leukopenia were developed. A cervical lymph node biopsy disclosed necrotizing lymphadenitis. We report a patient with necrotizing lymphadenitis followed by aseptic meningitis.

Background
There is a clear association between headache and sleep disturbance. However, the mechanism and causes are complex and poorly understood. The aim of this study is to compare the sleep quality in tension-type headache and migraine patients. Methods: We enrolled 49 tension-type headache (TTH) and 27 migraine patients. Two groups underwent the Pittsburgh Sleep Quality Index (PSQI), the Epworth Sleepiness Scale, Beck Depression Inventory, and State trait anxiety inventory. We analyzed these sleep[C3] and other psychiatric parameters according to headache type and headache features. Results: Global PSQI scores [10.82±5.10 for TTH vs. 10.52±4.31 for migraine, p-value=0.79], BDI[C4] [18.86±8.55 for TTH vs. 21.19±9.48 for migraine, p-value=0.28] and STAI[C5] [state; 51.04±10.32 for TTH vs. 52.48±11.92 for migraine, p-value=0.58, trait; 53.92±10.32 for TTH vs. 55.33±12.09 for migraine, p-value=0.59] showed no significant difference between two groups.Chronic migraine (CM) sufferers revealed shorter sleep duration [2.40±0.74 hours for CM vs. 1.42 ±1.08 hours for episodic migraine (EM), p-value=0.01]and higher STAI score comparing from EM (state; 57.87±11.71 for CM and 45.75±8.52 for EM, trait; 59.87±12.08 for CM and 49.67±9.83 for EM). Conclusions: In our study, there were no significant difference in sleep related parameters including PAQI, ESS and STAI between tension-type headache and migraine patients group. CM patients reported higher ESS and STAI scores comparing to EM patients.

The SUNCT refers to short-lasting unilateral neuralgiform headache with conjunctival injection and tearing. It is characterized by brief attacks of severe unilateral pain in the orbitotemporal region, associated w ith ipsilateral cranial autonomic disturbances. The syndrome is often m isdiagnosed as trigeminal neuralgia or cluster headache. A 60-year-old man had a right herpes zoster ophthalmicus. Four days later, he had first attack of pain deep in and around the right eye and temple w ith lacrimation. The pain w as sharp and throbbing, very intense and localized to the right orbital region. Reddening and w atering of the right eye consistently accompanied the pain. The headaches occurred sporadically throughout whole day, w ith an average of 20?30 individual paroxysms during 5-30 seconds. Attacks did not occur during sleep.
